About this role
Primary Healthcare Darlington is seeking a Clinical Pharmacist to join its Enhanced Health in Care Homes team in Darlington. The role is based at Forsyth House on Woodland Road and involves working as part of the PCN’s multidisciplinary group to support medication management in care homes across County Durham.
The position requires an independent prescriber or a commitment to completing prescribing training, with responsibilities centred on clinical medication reviews for residents with complex needs, including frailty, COPD, asthma, learning disabilities and autism. The post holder will supervise Pharmacy Technicians, contribute to medicines optimisation programmes, and address health inequalities through structured audits and quality improvement initiatives. The role also involves advising on antimicrobial stewardship, supporting CQC compliance, and collaborating with general practices, community pharmacists, and specialist services.
The post is permanent and available in full-time or part-time arrangements, with hours negotiable up to 37.5 per week. Pay is described as negotiable depending on experience, and the role is funded through the ARRS and the General Practice Network Contract Directed Enhanced Service (2026/27).
