Community GP

About this role The Community GP will be based at Amber Valley Health’s headquarters in Alfreton, Derbyshire, delivering clinical leadership for a dedicated team that supports housebound patients and those living in residential care across the ARCH Primary Care Network footprint. The post involves working closely with GP practices, community nurses and social-care partners to lead the day-to-day provision of acute home-visiting and urgent response services, with a particular focus on older adults experiencing frailty.

The role requires a qualified GP with current NHS registration, experience in community-based care for frail older people and a proven ability to work collaboratively across health and social-care boundaries. The post is offered on a permanent basis with a flexible full-time or part-time pattern, covering Monday, Thursday (essential) and Friday. Salary is negotiable within the NHS pay band for this level, and the successful candidate will have access to the NHS pension scheme and a programme of continuing professional development.
