About this role
The role is a permanent Band 4 position within the First Transitions Team, part of the Integrated Mental Health Teams across South Staffordshire. It is based at Foundation House, St Georges Hospital in Stafford, and involves intensive outreach and practical support for adults moving from forensic services into community-focused mental health care. The post requires frequent travel across a wide area, so a suitable vehicle is essential.

Essential experience includes significant work with adults experiencing mental health needs, collaboration in multi-agency settings, and experience in a forensic environment. Desirable experience comprises work with high-risk presentations and supporting service users in crisis-oriented community settings.
The role offers a salary of £28,392 to £31,157 per year, inclusive of the Agenda for Change pay scale, and provides permanent, full-time employment with opportunities for professional development.
