About this role The Norwich Urgent Community Response Therapy Team seeks an experienced Occupational Therapist or Physiotherapist to join its rapid response service. The post is based at Norwich Community Hospital on Bowthorpe Road and involves working across a seven-day week, with shifts typically from 08:00 to 18:00 and weekend rota cover. The role focuses on preventing unnecessary hospital admissions, supporting safe discharges and delivering evidence-based therapy to frail older adults, those with long-term conditions and end-of-life care needs.
The Trust requires a professional with a diploma or first-level degree in physiotherapy or occupational therapy, current HCPC registration and proven experience in complex assessments, care planning and multidisciplinary collaboration.
