Adult Hospital Liaison Practitioner

The role seeks an experienced Mental Health Practitioner to join the Adult Mental Health Hospital Liaison Team at Woodlands Unit, Rotherham Foundation Hospital Trust. You will provide urgent and emergency responses to mental health referrals from The Rotherham Foundation Trust, conducting full bio-psychosocial assessments and contributing to safe discharge planning. Working closely with TRFT staff, you will help develop their understanding of mental health and support training initiatives that place patients at the centre of their care.

Applicants must hold current UK professional registration as a Registered Mental Nurse or Registered Social Worker, with demonstrable post-registration experience in acute adult mental health and risk assessment. Essential skills include initiative, autonomous decision-making, and the ability to coordinate inter-agency approaches. Desirable attributes are strong report writing, IT proficiency, and experience of multidisciplinary team collaboration. The position is permanent, offered on a part-time, flexible basis within a 24/7 duty rota, and falls within Band 6 of the Agenda for Change pay scale, with a salary range of £39,959 to £48,117 per annum, pro rata.
