Care Co-ordinator

The Care Co-ordinator role is based in Tamworth and involves supporting clinical teams across five practices within Rural Warwickshire North Primary Care Network. Working with a patient cohort of over 35,000, the position combines administrative support with health promotion planning, acting as a liaison between patients and multidisciplinary teams. Responsibilities include managing patient review appointments, updating clinical records, coordinating care pathways, and contributing to Quality and Outcomes Framework initiatives. The role operates across multiple sites including Dordon, Polesworth, Water Orton, Nuneaton and Tamworth, requiring flexibility to travel between these locations.

Essential qualifications include a Level 3 administration credential or equivalent experience, with proven healthcare or social care background. Candidates must demonstrate strong IT proficiency, organisational accuracy, and familiarity with NHS systems such as EMIS. Key attributes include methodical record-keeping, data protection awareness, and the ability to manage competing priorities. Desirable experience encompasses project support, audit processes, and QOF delivery.
