Care Practitioner

The role seeks a Band 5 physical health nurse to join Mersey Care NHS Foundation Trust’s district nursing teams in Liverpool, covering an 8am-8pm seven-day week model across Knowsley’s neighbourhood footprint. The position operates within the Trust’s integrated community care programme, delivering evidence-based care in patients’ homes while adhering to Mersey Care’s values of safety, effectiveness and person-centred practice.
