Clinical Triage Practitioner

The role involves undertaking clinical triage of referrals into Children’s Specialist Services across Bath, North East Somerset, Swindon and Wiltshire, working as part of a multidisciplinary team to assess urgent cases, make decisions and communicate outcomes. The position is based in Swindon, with a hybrid arrangement and travel across the BSW locality as required, and operates on a 30-hour weekly rota of either 08:00-16:00 or 10:00-18:00. Candidates must hold current NMC or HCPC registration, have at least three years’ post-registration experience, and demonstrate specialist clinical knowledge, safeguarding awareness and experience of working with families, education services and voluntary organisations. The post offers a permanent Band 6 AfC salary of £39,959 to £48,117 per year, inclusive of NHS pension and Agenda for Change terms, together with access to My Reward Hub, wellbeing support, eLearning and development pathways.
