Clinical Triage Practitioner

Clinical Triage Practitioner HCRG Care Group operates from Swindon in Wiltshire, offering a permanent position that carries a salary of £39,959 to £48,117 per year. The role is based within the Children's Specialist Services team and involves undertaking clinical triage of referrals, including urgent cases, and coordinating care across health, education, social care and voluntary sectors. The successful candidate will work as part of a multidisciplinary team, provide specialist clinical advice, and ensure that children, young people and their families are directed to appropriate services through a coordinated care pathway. Travel across Bath and North East Somerset, Swindon and Wiltshire is required, with hybrid home working available and hours of 37.5 per week on a rota that includes either 8:00am to 4:00pm or 10:00am to 6:00pm.
