About this role
An opportunity exists for a Discharge and Flow Clinician to join the Patient Flow and Discharge Team at Great Western Hospital in Swindon, Wiltshire, working as part of a multidisciplinary seven-day service. The role involves coordinating timely, safe discharge pathways across inpatient services, undertaking holistic assessments, and liaising with consultants, ward teams, community providers, social care and voluntary organisations to ensure continuity of care. The post is set within HCRG Care Group’s discharge service, which aims to improve patient experience, reduce delays and support positive health outcomes.
Essential requirements include registration as a nurse, allied health professional or social worker, experience in discharge planning or transitional care, and knowledge of the Care Act, Mental Capacity Act, safeguarding and information governance. Effective communication, negotiation and conflict-resolution skills are required, as is the ability to manage complex caseloads and work collaboratively across agencies. Desirable experience includes quality-improvement work, supervisory or mentorship roles, and familiarity with continuing healthcare processes.
