Community Case Manager

The role is for a Community Case Manager within the Urgent Community Response Directorate’s Virtual Ward Team at Sandwell and West Birmingham NHS Trust in West Bromwich. The position involves managing a caseload of patients with complex long-term conditions and social risk factors across four virtual wards – respiratory, frailty, EpiCentre and cardiology – with the aim of preventing unnecessary hospital admissions. Reporting to a senior Community Matron, the postholder will act as an autonomous practitioner, assess and intervene in acute illness or disease exacerbation, and coordinate care across primary and secondary services.

Applicants must be a Registered General Nurse for adults, hold a recognised teaching or assessing qualification, and be a qualified or prospective nurse prescriber. Essential experience includes recent work in community or acute care, specialist knowledge of long-term conditions, documented CPD, and proven ability to work in a multidisciplinary team while leading junior staff. A full UK driving licence and access to a suitable insured vehicle are required. The post is permanent, full-time, falls under Band 6 of the Agenda for Change pay scheme with a salary of £39,959 to £48,117 per year, and closes on 19 October 2026.
