Community Mental Health Practitioner (AA&HTT)

The role is based at The Crystal Centre in Chelmsford, where the successful candidate will work within the new Mid and South Essex Older Adult Admission Avoidance & Home Treatment Team. This part-time position involves 22.5 hours per week supporting older adults with frailty and functional mental health needs, using a Home First approach to prevent unnecessary admissions and deliver home-based treatment. The post is part of a developing service that collaborates with existing community teams and dementia avoidance services across the region.

Applicants must be a registered mental health nurse or occupational therapist with relevant experience in older-adult mental health and dementia care. Essential qualifications include professional registration in the UK and evidence of recent development in contemporary mental health practice. Desirable criteria comprise a degree-level education, non-medical independent prescribing and familiarity with risk assessment tools. The role offers a permanent contract within Band 6 of the Agenda for Change pay scale, ranging from £39,959 to £48,117 per annum, plus benefits such as 27 days of holiday (rising to 33 after ten years), a pension of up to 14.5 % and a £8 K relocation package.
