About this role
The role is a permanent Community Nurse Practitioner position within the Norfolk Forensic Community Learning Disability team, based at Little Plumstead Hospital in Norwich. The post is part of the Hertfordshire Partnership University NHS Foundation Trust’s specialist forensic learning disability service, working across both inpatient and community settings to support adults with learning disabilities and complex needs. The successful candidate will lead care planning for a caseload, collaborate with medical, psychology and therapy staff, and ensure safe, evidence-based practice in line with national and local policies.
