About this role

The Warrens is a community nursing team based in Thingwall, Merseyside, offering a permanent full-time position within the NHS Agenda for Change. The role involves a 37.5-hour week across seven days, with shifts that require a valid driving licence for work travel. Candidates will conduct holistic assessments of physical, social, emotional and environmental needs for patients managing long-term conditions and frailty, and will advise patients and carers to promote independence. The post is autonomous, operating under the direction of the Clinical team leader and in line with the NMC Code of Conduct, and it requires evidence-based practice, strong organisational skills and the ability to communicate clearly with multidisciplinary teams.

Essential qualifications include registration as a General Nurse at degree level with the NMC and recent professional development, while desirable experience encompasses community nursing and mentorship training. Essential attributes are effective interpersonal and written communication, computer literacy and a solid grasp of national health and social-care initiatives, as well as knowledge of conditions such as diabetes, COPD, dementia and palliative care. The salary ranges from £32,073 to £39,043 per annum, and applications must be submitted by 15 October 2026 via the original advertisement.
