About this role

About this role The position is based in Braintree, Essex, and involves working as part of the multidisciplinary team within the Mid Essex Dementia and Frailty Pathway. The role focuses on supporting older adults and patients with dementia who also have functional mental health needs and frailty, delivering physical health assessments and promoting wellbeing across community settings.

The role requires a valid UK driving licence and access to a car for travel across Mid Essex sites, as well as IT literacy and strong oral communication skills. Essential qualifications include an NVQ Level 3 or willingness to work towards it, completion of the Care Certificate within the first 12 weeks, and experience in mental health support. Desirable criteria include GCSEs in Maths and English, experience in older adult mental health, and familiarity with systems such as System One or PARIS.

The post is permanent, full-time, and operates Monday to Friday from 9:00 to 17:00. The salary ranges from £25,760 to £27,476 per year, with benefits including 27 days of holiday (rising to 33 after 10 years), an NHS pension of up to 14.5%, and access to staff discounts and relocation support.
