Mental Health Practitioner - Primary Care

About this role The position is based at Pool Health Centre in Cornwall and forms part of the North Kerrier West Primary Care Network. You will work as a Mental Health Practitioner within a multidisciplinary primary care team that includes GPs, social prescribers, community health and wellbeing workers and secondary care colleagues. The role involves assessing and triaging patients with mental health concerns, managing long-term conditions, conducting medication reviews and delivering evidence-based interventions. You will also contribute to the development of innovative mental health pathways, support proactive care for vulnerable groups and collaborate with the wider PCN to reduce health inequalities.

The role requires registration as a qualified mental health professional with relevant clinical experience in primary or community mental health services. Experience in autonomous practice and teamwork within a multidisciplinary setting is essential, as is familiarity with care planning, risk assessment and safeguarding procedures. The position offers a permanent contract with flexible working options, a salary in the region of AFC Band 6 depending on experience and support for non-medical prescribing where appropriate.
